Mediastinal synovial sarcoma invading the right inferior pulmonary vein and left atrium

      Abstract

      We report a rare case of mediastinal synovial sarcoma. The tumor was a 40-mm mass in the posterior mediastinum, infiltrating the inferior lobar vein and left atrium and located close to the esophagus. This patient was scheduled to undergo tumor extirpation combined with resection of the right lower lung lobe and left atrium. Considering the reconstructive route after esophagectomy, the chest was opened through an anterolateral incision. No invasion into the esophagus was present, and complete removal was achieved by combined resection of the right lower lung lobe and left atrium. The resected specimen contained the SS18-SSX1 fusion gene, leading to a diagnosis of synovial sarcoma.
